 How to qualify for your Arizona Medical Marijuana Card.

The following are the steps in the qualification process.

You must have a medical condition that qualifies you under Arizona state law. If you suffer from any of the conditions below, you could benefit from the use of Medical Marijuana and should Get your Medical Marijuana Card:

  • Agitation of Alzheimer’s Disease
  • Amyotrophic Lateral Sclerosis
  • Cancer
  • Crohn’s Disease
  • Glaucoma
  • Hepatitis C
  • HIV/AIDS
  • PTSD
  • Cachexia or Wasting Syndrome
  • Severe Nausea
  • Severe and Chronic Pain
  • Seizures
  • Severe and Persistent Muscle Spasms

Step One: Medical History

The first step in the application process is documentation of your medical condition. The AZ Medical Marijuana certification form states that the certifying physician has reviewed medical records within the last year. You can simply provide a copy of your most recent medical chart notes or most patients will utilize our pain evaluation services. Our in house physicians can help to document your medical history and generate the required documentation needed for most pain issues.  Patients must have records dated within the year to qualify, even for medical conditions that are permanent ie.. HIV, glaucoma.

Please note that faxed records are limited to no more than 25 pages. We only require records dated withing the last year and no large files like X-rays or MRI images are needed. We prefer to have your records faxed for efficiency purposes and reduced expense in comparison to mailing.

Step Two: Physician Review

We will have one of our qualified physicians review your medical history to ensure the state’s requirements for certification are met. We will phone you once your records arrive, verify you have what is needed, and schedule your appointment.  If you do not have access to medical records, we can verify your qualifying condition over the phone and set up your appointment with our doctors for pain evaluation.

Step 3: Certification Appointment

The Medical Marijuana Card certification appointment usually takes about an hour to complete. Our doctors will interview you and review/document your medical history, perform a physical exam, then complete the state certification form. If you don’t qualify based on your medical history, we will not charge you for the appointment. If you don’t get certified, you don’t pay.

Our current special: $130 or $125 if you pay cash. 

Step 4: Application for your Arizona Medical Marijuana Card

In order to obtain your Arizona Medical Marijuana Card, you must upload the following information to the Arizona Department of Health Services website: They charge $150 for most patients or $75 if on the Supplemental Nutrition Assistance Program.

  • Current Photograph
  • Physician’s Certification Form
  • Patient Attestation
  • AZ driver’s license, state ID, or US Passport.
  • SNAP or Food Stamp documents if applicable.
